Important Information about the HIPAA Privacy Regulations

Dear Valued Patient,

The United States Department of Health and Human Services has issued regulations establishing strict standards in how Pharmacies, like United Medco Pharmaceuticals, may use and disclose your medical records. These regulations are effective as of April 14, 2003.

Included is a notice that explains the Rules in detail. In general, the regulations require United Medco Pharmaceuticals to secure all medical information so that it is not readily accessible or available to those who do not need access to it. We will be able to discuss your information with you directly, but we will require your authorization to discuss this information with third parties, such as your spouse. In order to obtain this authorization, we must have a written authorization on file for you and your spouse, if married. (There is an exception in the Rules allowing parents to obtain information from us concerning their minor children.) We have included an Authorization Form, additional forms can be obtained by photocopying, and encourage you to take a moment to review, complete, and return as many forms as needed to:

Once authorized by you, we are permitted to disclose necessary information about you to the person(s) that you have designated on the Authorization Form(s).

Our commitment to protecting your privacy is not new. We will continue to safeguard your privacy and the confidentiality of the information provided to us related to your pharmaceutical needs.

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

This notice of Privacy Practices describes how we may use and disclose your protected health information to carry out treatment, payment or health care operations and for other purposes that are permitted or required by law. It also describes your rights to access and control your protected health information. "Protected health information" is information about you, including demographic information, that may identify you and that relates to your past, present or future physical or mental health or condition and related health care services.

We are required to abide by the terms of this Notice of Privacy Practices. We may change the terms of our notice, at any time. The new notice will be effective for all protected health information that we maintain at that time. Upon your request, we will provide you with any revised Notice of Privacy Practices by accessing our website www.unitedmedco.com, calling the pharmacy and requesting that a revised copy be sent to you in the mail or asking for one at the time of your next visit.
